This report may contain information regarding my past education, past employment, and general reputation from public record sources. I understand that I have a right to request additional disclosures regarding the nature and scope of the investigation.

By signing the form below, you authorize All Plastics to obtain or cause to be obtained a background check. If hired, this authorization shall remain in effect and serve as continuing authorization for All Plastics LLC to obtain background reports at any time during your employment with All Plastics LLC.
